Fire in Pilot Plant
Following a power failure, there was a fire in the Pilot Plant when the power was restored
The power failed in the Pilot Plant. The back up diesel generator failed to start up and therefore the back up systems all failed, and the agitator on a vessel with refluxing solvent stopped. When the power was restored, the vessel boiled up rapidly and solvent was lost. The ignition of the solvent was put down to a nearby wiring failure.
- Back up systems are only effective if the work! Therefore they need to be regularly maintained and tested.
- If back-up systems cannot be trusted 100% then alternative methods of emergency action must be provided - cooling the reactor would obviously have helped in this case.
