Minor incident made worse by maloperation
After testing a slurry in a 1000 litre IBC, it was found to be out of specification due to the overcharging of one component. Disposal was therefore necessary, and the plant manager decided to charge bags of portland cement to solidify the material, so it could used as landfill.
Naturatally the temperature rose uncontrollably to over 100°C, the material boiled violently, and was ejected from the IBC.
