Phosgene Release

Incident

Phosgene was released from a scrubber complex.

The Investigation

Three caustic scrubbers were used in series, the operators checked the scrubber contents and recharged them as required. This eventually led to a biuld up of carbonates and bicarbonates, with the inevitable formation of solids. These solids blocked the pipework, causing the phosgene to be released.
