Misidentifaction of Services causes batches to fail

Incident

Two failed process runs in a fume cupboard warrented further investigation

Problem

Cause was evetually traced to the nitrogen supply in the fume cupboard delivering compressed air. The reaction that was being carried out was air and water sensitive. It transpired that work on the services to the cupboard had resulted in a cross over of the nitrogen and air lines, which had been noticed but not rectified at the time

Outcome

A change to the PTW procedure was therefore initiated.
