Carius tube broke up on removal
Incident
The title says it all!. Dismantling following an exotherm in calorimeters is always problematic, and not just limited to Carius tubes. ARC bombs have also been known to be problematic as well, as a runaway can often block the narrow feed-through-tube with black foam, which later sets, leaving a bomb under pressure, which cannot be vented, except by careful removal.
